The Loddon Children’s Health and Wellbeing Local (LCHWL) provides a range of health and wellbeing services to children up to 11 years of age who may be experiencing some challenges including, developmental, emotional, relational or behavioural concerns. Services are available to families across the shires of Campaspe, Central Goldfields, Loddon, Macedon Ranges, Mt Alexander and Greater Bendigo. The LCHWL is aninnovative model of care that is co-designed with paediatric service providers and people with lived experience, to provide a system response that improves access to primary and tertiary care across the Loddon region. Services provided through the LCHWL include paediatric health, mental health support, parenting support, specialist assessments, allied health, family services and care coordination. The LCHWL has recently been established in response to the Royal Commission into Victoria’s Mental Health System.

We are currently seeking application for the recently created role of - Children's Therapeutic Specialist. This position is a vital role aimed at providing support and therapeutic services to children aged 0 to 11 years and their families. The position operates within the Echuca Regional Health (ERH) catchment and is part of a larger care system called the Loddon Children’s Wellbeing Local (LCHWL).

Key responsibilities of this role include:

• Providing accessible and coordinated services: The specialist will work closely with children and their families to provide accessible therapeutic services that address the children's emotional, psychological, and developmental needs.
• Integrated care approach: The specialist will work as part of a multidisciplinary team within the LCHWL, collaborating with other professionals and organizations to offer comprehensive and integrated care.
• Family-focused care: The role involves engaging with families to ensure they are supported in meeting their child's needs and to promote healthy family dynamics.
• Early intervention and prevention: The specialist will play a role in identifying and addressing potential issues early on, offering preventive measures and early intervention strategies to promote the wellbeing of children and their families.
• Community outreach and advocacy: The position may involve engaging with the community to promote awareness and understanding of children's therapeutic needs and advocating for resources and support.

• Proven experience working in similar setting (including at least 2 years in a paediatric specific role)
• Relevant tertiary qualifications in a mental health discipline
• Excellent time management and organization skills

Key responsibilities of the Child and Family Intake Coordinator include:

• Intake and assessment: Handling initial inquiries from families and conducting assessments to determine the level and type of support needed.
• Strength-based and family-centered approach: Applying a holistic, child- and family-centered approach to care, focusing on the strengths and unique needs of each child and family.
• System navigation: Assisting families in navigating the care system, providing guidance and support to access appropriate services and resources.
• Coordination and collaboration: Working with other professionals and organizations within the LCHWL consortium to coordinate care and provide integrated support.
• No wrong door approach: Ensuring a seamless experience for children and families, regardless of the point of entry into the system, and facilitating access to necessary services without barriers.
• Follow-up and continuity of care: Following up with families to ensure continuity of care and ongoing support as needed.

The Subacute Intake Coordinator collaborates with the Bed Manager, Rehab NUM and Rehab Clinical Health Lead to develop and maintain streamlined intake systems for the Rehab Ward in the context of the broader intake system for bed based services at ERH. The role also has overall responsibility for assessing all referrals and coordinating admissions to the Rehab Ward. The Rehabilitation Ward combines contemporary inter-professional practices and fantastic patient outcomes in a brand new state of the art rehabilitation unit. 

The Rehabilitation Ward provides care for patients in four care types: Rehab, GEM, TCP and Palliative Care and therefore ensures a diverse and dynamic working environment. 

Key responsibilities include:

• Ensure the systems for Rehab Ward intake and transitioning patients to the Rehab Ward are streamlined, efficient and patient centred.
• Work collaboratively with the Bed Manager to ensure the Rehab intake system is consistent with other intake systems for ERH hospital bed based services.
• Ensure and support staff to use practices consistent with the ERH system for the referral of patients to bed based services and transitioning patients from one service to another.
• Actively input into the development of the Bed Manager KPIs suite to ensure the effectiveness of intake and transfer systems are routinely monitored and assessed.
• Coordinate the intake of individual patients to the Rehab Ward.
• Attend care coordination meetings.

An exciting opportunity has arisen across 2 departments within Echuca Regional Health.  Due to vacancies across the IPAC and Physiotherapy department we are giving applicants to opportunity to apply for Full Time hours. This role will combine a permanent part time role in Complex Care Physiotherapist (Grade 1 or 2) in our IPAC team and fixed term contract within our Physiotherapy team. This gives applicants the best of both worlds and the opportunity for full time or part time work.   

Key responsibilities within IPAC include:

• providing support to clients and managing a heart failure group  
• case management of clients with chronic disease
• development of health action plans
• management of a clinical caseload and delivering expert physiotherapy assessments    

Key responsibilities within the Physiotherapy team include:

• Maintaining a clinical load appropriate to level of experience                
• Contributing to the day to day operation / functioning of the Physiotherapy department
• Senior Physiotherapist involved in the supervision and mentorship of junior physiotherapists

The role within the Physio team will include work across multiple areas including the below.

• Working as part of the specialist clinical teams, involved in the assessment and management of a variety of patients with complex issues.
• Working as part of the community rehabilitation team (attending weekly meetings)
• Management of inpatients on the maternity ward.
• Paediatrics, NDIS and community.
• Inpatient wards including orthopaedic and general surgery, maternity, general medicine and high dependency unit

 Both teams require someone who has:

• Qualifications in physiotherapy   
• Proven experience of the independent management of a clinical case load   
• High-level awareness of chronic disease management        
• Strong leadership skills in a multidisciplinary team
